We ensure that the care package for each Service user is designed specifically for that individual. Our main aim is to retain and enhance on any skills that each service user may have acquired.

The first stage is an initial contact which may be made by telephone, fax, e-mail or in writing. At this point we will ask for as much information about the prospective service user as is possible. This can include: an assessment report; or clinical report.

The next stage is the assessment process. Meadow View Residential Homes carries out a very thorough and extensive assessment process. An assessment is a long process which aims to collect as much information about the individual as possible to help us find out their needs and what service can be put together by the organisation to meet the individual’s needs. This process may include family members, people who work on a day to day basis with the service user and other professionals.

This process enables us to create a pen picture of an individual. Our belief is that the passing on of information/communication is the vital key to a successful placement.
